Advanced: masseter toxin for jawline slimming & bruxism — dosing and safe technique?

Adding masseter to my menu after advanced training. What doses are people using and how are you keeping it safe and symmetrical?

Masseter is a rewarding advanced area but respect the anatomy. Dose: commonly ~20–30 units per side (Botox units) to start, individualised to muscle bulk — big male masseters may need more, and you can review and add. Palpate the muscle while the patient clenches to map its true extent. Technique: inject deep, down to bone, across 2–3 points within the safe zone — stay inside the muscle, below a line from the tragus to the corner of the mouth, and behind the anterior border. The key safety point is staying posterior/inferior enough to avoid the risorius and the other perioral elevators, or you'll get an asymmetric or awkward smile. Counselling: results build over several weeks (it's partly muscle atrophy over time), first-timers can get transient chewing fatigue or a temporary feeling of bulging, and repeat sessions are usually needed for slimming. Great for bruxism/TMJ symptom relief too — set expectations that slimming is gradual.

  • From the dental side — I get a lot of bruxism referrals and masseter toxin genuinely helps symptoms. Just be clear it complements, not replaces, a splint where one's indicated.

  • The 'palpate while they clench and stay behind the anterior border' advice is what saved me from risorius-related smile asymmetry. Map every patient individually.
