In practice many clinics reconstitute and use within the same session/day, keep it refrigerated per the SPC, and don't stockpile reconstituted vials. Label with the time of reconstitution, keep the cold chain, and if you're ever unsure whether a vial has been handled correctly, don't use it — a wasted vial is cheaper than an under-dosed or unsafe treatment.
